Saline Lock Step One

Verify Orders; Check for Accuracy; Look up in Drug Ref. Book (side effects, time to infuse medication, & incompatibilities); pt. weight; allergies.

Saline Lock Step Two

Perform Calculations

Saline Lock Step Three

Gather equiptment: 3 needless syringes, blunt cannulas, NS vial, Alcohol pads, labels, and medication.

Saline Lock Step Four

Hand Hygiene, Don Clean Gloves, Open supplies (do not contaminate).

Saline Lock Step Five

Check Medication again; draw up calculated amount of medication with needless system and label the syringe; Draw up two saline flush syringes and label these syringes. (Use the needless system for all three syringes).

Saline Lock Step Six

Enter pt.'s room with MAR; and three labeled syringes; confirm identity with two identifiers; Into. self; Explain medication; ask pt. about medication; Ask pt. if have any questions; Confirm allergies; and Provide for Privacy.

Saline Lock Step Seven

Hand Hygiene & Don Clean Gloves.

Saline Lock Step Eight

Assess the IV site for: phlebitis (swelling); warm to touch; redness; infiltration; drainage; and pain.

Saline Lock Step Nine

Open clamp located between the catheter and the port on the extension set; clean the port with an alcohol wipe; and allow to dry.

Saline Lock Step Ten

Connect the saline flush syringe with the IV port; ASPIRATE FOR BLOOD (if present and no other signs of infiltration are present continue with flush); Withdrawl and dispose of syringe.

Saline Lock Step Eleven

Again clean the IV port with an alcohol wipe and allow to dry; Connect medication syringe with IV port; infuse medication over appropriate time period; Again clean IV port with an alcohol wipe and allow to dry.

Saline Lock Step Twelve

Connect the second saline flush to IV port; administer flush; withdraw the needless system and dispose of properly; & re-clamp saline lock.

Saline Lock Step Thirteen

Reposition pt, provide for safety; remove gloves, & perform hand hygiene.
