NUTRITIONAL ASSESSMENT

Identify if a patient is receiving enough nutrient intake to provide for daily bodily requirement and increase metabolic need
Degree of balance between intake and normal recommended requirement

FACTOR OF NUTRITION ASSESSMENT

-ID a malnourished client
-ID Risk factors (diabetes, mental status and age, diet, financial status, cultural, self perception, occupational)
-obtain data for nutritional plan

OBJECTIVE HEALTH DATA

-Skin (smooth, no rashes, bruises, flaking, warm, dry, intact)
-Hair (shiny, firm, healthy scalp)
-Gums (pink and firm)
-Nails (smooth, pink, clean)
-Eyes (clear, shiny)
-Tongue (no swelling, pink)
-Lips (smooth, moist, no swollen)

RISK FACTORS FOR NUTRITIONAL DEFICITIS

Weigh under 80% or above 120%
-Unintentional weight loss
-Serum albumin under 3.8 g/dl
-Lymph count under 1500
-History of illness

DIETARY HISTORY

-24 hr dietary recall

Hemoglobin/Hematocrit
Hemoglobin: iron deficiency anemia
Male 14-18
Female 12-16
Hematocrit: measures cell volume and iron status
Male- 37-49% Female 36-46%

HDL/LDL

Cholesterol- fatty plaque in artery
120-200 (male/female)
HDL (good cholesterol)
male 35-70 female 38-85
LDL- should always be less than 130

TRIGLYCERIDES

Used to screen for hyperlipidemia
should be btw 40-200
Is bad if high because the clog arteries

TOTAL LYMPH (TLC)

Test for immune function
important indicator of visceral protein status and therefore of cellular immune function
